Collection Site Network Request

By checking "I Agree", you are indicating that your facility IS compliant with the DOT 49 CFR Part 40 collection procedures and training requirements. Additionally, your facility agrees to maintain the documentation and record keeping required under 49 CFR Part 40; and to provide this documentation to Pipeline Testing Consortium, Inc. immediately when requested to do so.

Furthermore, you are also agreeing to submit invoices for drug and/or alcohol collections within 12 calendar months of the service date and accept that PTC does not pay for collections older than 1 year. We advise all in-network collection sites bill within 30 days of collections performed. If there is an outstanding invoice, we request that you send us a copy of the invoice and call 800-294-8758, ext. 464 to confirm it was received.
